BIS registration documents, filled in for your product

What the kit writes

Registration (CRS, Scheme II)

  • Checklist of every document and how to issue it
  • Field sheet for the portal application (Form I)
  • Nomination of the Authorised Indian Representative (Form IV), for factories outside India
  • Affidavit cum undertaking for stamp paper (Form III A, B or C, picked for you)
  • Undertaking on the test report (Form II)
  • Brand authorisation letter, when someone else owns the brand
  • Test request sheet and labelling sheet (Standard Mark, R-number)

ISI mark licence (Scheme I)

  • Checklist, including Forms IX to XI for factories outside India
  • Field sheet for the application (Form V) with its 13 documents
  • Self-evaluation cum verification report
  • Machinery, test equipment, levels of control and test report forms (Forms I to IV)
  • Nomination of the Authorised Indian Representative (Form VI), for factories outside India
  • Brand authorisation letter and labelling sheet

Laboratory tests, BIS's fees, stamp paper and notary, and the portal application stay with you. Documents are in English, as BIS's forms are.

Scheme IV, Scheme X and upcoming orders are not covered: their products are not offered below.
